Just like the stock stem. Press fit in the bottom triple, a pair of bearings and a bf nut on the top trip. The 'adjustability' comes from custom inserts in the triples. No. Um, ahem only an idiot can do that...  Seriously tho, if it's put together correctly there's no way. There's a shoulder in the bottom of the stem that prevents it pulling through and a BFN on top preventing anything coming out of the head tube. The adjustable plugs are tapered and directional in their respective cut outs and there's pinch bolts everywhere.
